By signing up for the HealthCare Interactive Affiliate Program, you agree to the following terms and conditions:

• There is no cost or obligation to be a part of the affiliate program. You may stop participating at any time.
• A referral fee of 10% will be paid on completed sales, not on clicks. The fee is calculated on the price the customer actually pays after any discounts or special offers have been applied. Examples:
  
  
1. A customer pays full price for a 1-year site license to CARES for $799.95. You will receive 10% of $799.95, for a total referral fee of $79.95.
2. A customer purchases a 1-year site license to CARES for $799.95, but receives a 20% discount through a special offer. The total purchase price the customer pays is $639.96, and you will receive 10% of this price, for a total referral fee of $64.00.
3. A customer purchases a Savvy Caregiver 4-DVD set for $49.95 and a Savvy Caregiver Homework CD-ROM for $29.95, for a total purchase price of $79.90. You will receive 10% of $79.90, for a total referral fee of $7.99.


• Referral fees are paid quarterly for all account balances that exceed $100.00. Payments on all accounts will be made yearly regardless of account balance. Fees are paid within 30 days of the end of each calendar quarter. For example, account balances that exceed $100 for first quarter (January, February, and March) will be paid by April 30.
• Only approved HCI banners, graphics, and HTML code may be used.
• Affiliates can log into their control panel and view their account activity at any time.
• HCI may cancel an Affiliate at any time. Completed referral fees will be paid within 60 days of termination. Affiliates may also cancel from the program at any time. Completed referral fees will be paid 60 days after resignation.
